What Is Telecare Connect, and How Can It Help You?

Keep your blood pressure or blood sugar readings on your phone. See how they change over time. Have them ready for your next doctor visit.
Where do you keep your blood pressure or blood sugar readings?
You check your blood pressure or blood sugar at home.
But where do you save the readings?
Do you write them in a notebook? Take photos? Or sometimes forget to write them down?
Now think about your next doctor visit.
Your doctor may ask to see your recent readings.
Will you be able to find them?
What if you could take out your phone and see your readings in one place?
That is what Telecare Connect helps you do.
You can save your readings, see how they change over time, and show them to your doctor.
Your readings, all in one place
Most of us carry our phones every day.
With Telecare's app, your saved readings are close at hand.
Here are three simple things you can do.
1. Save your readings
After checking your blood pressure or blood sugar at home, open Telecare's app.
Enter the reading and save it.
You can also save your weight.
You use your own home testing device. It does not need to connect to the app.
2. See how your readings change
Want to know what your blood pressure was last week?
Open the app and tap My Numbers.
You can see your 10 most recent readings and a chart showing how your readings have changed over time.
For blood pressure, you can also see your averages for the last 7 and 30 days.
For blood sugar, the app can show an average of readings marked as fasting.
You can also look back at your weight readings and see how your weight has changed.
You do not need to search through old notes or photos.
3. Show your readings to your doctor
At your next visit, take out your phone.
Open My Numbers and show your saved readings.
Your doctor can look at them with you and help you understand what they mean for your care.
Your readings stay close at hand, ready when you need them.
Why did we create Telecare Connect?
Life gets busy.
You may be working, caring for your family, or taking care of things at home.
Keeping track of blood pressure or blood sugar can be one more thing to remember.
Telecare Health is a Belize-based company founded by a physician with more than 20 years of clinical experience.
We created Telecare Connect to make it easier to keep your readings in one place and look back at them.
Our approach is built around three simple ideas:
Save your readings. See how they change. Share them with your doctor.
We also help you learn how to use the app.
Telecare does not replace your doctor. Your doctor remains responsible for your medical care.
Which program can you choose?
We offer programs based on what you want to track.
PressureCare Connect
For people who want to keep track of their blood pressure.
You can save your blood pressure readings, see trends, and look back at your averages.
DiabetesCare Connect
For people who want to keep track of their blood sugar.
You can save blood sugar readings and see how they change over time.
VitalCare Connect
For people who want to track both blood pressure and blood sugar.
Both types of readings can be saved in the same app.
WeightCare tracking is included at no extra cost with these programs.
You can save your weight and see how it changes over time.
How do you get started?
Step 1. Tell us what you need
Start with five short Program Fit questions.
They help us learn how you keep track of your readings and what help you may want.
This is not a medical test or a sign-up form.
Step 2. Learn about Telecare
Our team may contact you to explain the programs, the cost, and how they work.
You can then decide whether you want to apply to join.
Step 3. Start using the app
Once you have enrolled and received app access, we help you get started.
We show you how to enter your first reading and find your saved numbers.
Then you can keep adding readings whenever you check them at home.
What will you need to use the app?
You will need:
A phone with internet access and an email address.
An upper-arm blood pressure monitor if you track blood pressure.
A blood sugar meter if you track blood sugar.
A scale if you want to track your weight.
You enter your readings yourself. Your devices do not need to connect to the app.
Questions you may have
Can someone help me get started?
Yes. We help you set up the app, enter your first reading, and find your saved numbers.
Does someone watch my readings all the time?
No. Telecare is not a continuous monitoring or emergency service. Some readings may trigger automated alerts, but that does not mean a clinician has reviewed them. No alert does not mean you are safe.
Does Telecare replace my doctor?
No. Keep seeing your doctor and following your care plan. The app does not diagnose illness, prescribe medicine, or change your treatment.
What if I feel very unwell?
Get urgent medical care. Do not wait for an app alert or a WhatsApp reply.
Ready to keep your readings together?
Think about your next doctor visit.
Instead of looking through old notes or photos, you could take out your phone and show the readings you have saved.
Start by telling us how you keep track of your blood pressure or blood sugar today.
Answer five short questions. Sending your answers does not enroll you in Telecare or require payment.
Telecare Connect helps you track readings and learn to use the app. It does not replace your doctor, provide continuous monitoring, or offer emergency care.
